With due respect, I would like to inform you that I have been suffering from fever since last night. The doctor adviced me to take rest for at least 5 days. So I need 5 days leave from 11 February 2022 to 15 February 2022 instant. In truth of my statement, I have presented the prescription of the doctor along with this application for your kind consideration.
Therefore, I expect that you would be kind enough to grant me leave for those days.
